Pharmacologic Category
Antiulcer Agents and Acid Suppressants; Histamine H~2~-Antagonists. (ATC-Code: A02BA03).
Mechanism of action
Famotidine works by reducing the amount of acid in the stomach thereby reducing pain, and allowing the ulcer to heal, and through a competitive inhibition of histamine at H~2~ receptors of gastric parietal cells, which inhibits gastric acid secretion.
Therapeutic use
Short-term treatment of active duodenal ulcer. Maintenance of healing and reduction in recurrence of duodenal ulcer. Treatment of Zollinger-Ellison syndrome, multiple endocrine adenomas. Short-term treatment of active benign gastric ulcer, symptomatic gastroesophageal reflux, esophagitis, (including erosions or ulcers) in gastroesophageal reflux. Short-term self-medication for relief of heartburn, acid indigestion, and sour stomach.
Pregnancy and lactiation implications
Crosses placenta. Use only if clearly needed. Not recommended during lactation.
Unlabeled use
Part of a multidrug regimen for H. pylori eradication to reduce risk of duodenal ulcer recurrence. Stress ulcer prophylaxis in critically-ill patients. Symptomatic relief in gastritis.
Contraindications
Hypersensitivity to famotidine, other H~2~ antagonists (e.g. cimetidine, nizatidine, ranitidine), or any component of the formulation.
Warnings and precautions
Reversible confusional states (increased age (>50 years) and renal or hepatic impairment thought to be associated). Relief of symptoms does not preclude the presence of a gastric malignancy. Use with caution in moderate-to-severe renal impairment (CrCl <50 mL/minute). Multidose vials for injection contain benzyl alcohol, associated with «gasping syndrome» in neonates. Not for use with other acid reducers. Some dosage forms may contain aspartame, metabolized in the GI tract to phenylalanine. Administration of H~2~-receptor antagonists associated with increased risk for developing certain infections (e.g. community-acquired pneumonia).
