Fosfomycin Tromethamine: C~3~H~7~O~4~P C~4~H~11~NO~3~. Mw: 259.19. (1) Phosphonic acid, (3-methyloxiranyl)-, (2R-cis)-, compd. with 2-amino-2-(hydroxymethyl)-1,3-propanediol (1:1); (2)(1R,2S)-(1,2-Epoxypropyl)phosphonic acid, compound with 2-amino-2-(hydroxymethyl)-1,3-propanediol (1:1). CAS-78964-85-9 (1991).

Pharmacologic Category

Urinary Anti-infectives. (ATC-Code: J01XX01).

Mechanism of action

As a phosphoric acid derivative, fosfomycin inhibits bacterial wall synthesis (bactericidal) by inactivating the enzyme pyruvyl transferase, critical in synthesis of cell walls by bacteria. Tromethamine salt is preferable to calcium salt due to its superior absorption.

Therapeutic use

Single oral dose in treatment of uncomplicated urinary tract infections in women due to susceptible strains of E. coli and Enterococcus.

Pregnancy and lactiation implications

There are no adequate, well-controlled studies in pregnant women. Use not recommended during lactation.

Unlabeled use

Multiple doses investigated for complicated urinary tract infections in men.

Contraindications

Hypersensitivity to fosfomycin or any component of the formulation.

Warnings and precautions

Prolonged use may result in fungal or bacterial superinfection, including C. difficile-associated diarrhea and pseudomembranous colitis.
