Pharmacologic Category
Antidiabetic Agents; Sulfonylureas. (ATC-Code: A10BB12).
Mechanism of action
Stimulates insulin release from pancreatic β-cells. Reduces glucose output from liver. Insulin sensitivity increased at peripheral target sites.
Therapeutic use
Used as monotherapy as adjunct to diet and exercise for management of type 2 (non-insulin-dependent) diabetes mellitus in patients whose hyperglycemia cannot be controlled by diet and exercise alone. May also be used in combination with one or more other oral antidiabetic agents or insulin as adjunct to diet and exercise for management of type 2 diabetes mellitus in patients in whom adequate glycemic control cannot be achieved with diet, exercise, and oral antidiabetic agent monotherapy. Glimepiride is used in fixed combination with rosiglitazone in patients with type 2 diabetes mellitus who are already receiving rosiglitazone and a sulfonylurea separately or who are inadequately controlled on a sulfonylurea or rosiglitazone alone. Also used in fixed combination with pioglitazone in patients with type 2 diabetes mellitus who are already receiving pioglitazone and a sulfonylurea separately or who are inadequately controlled on a sulfonylurea or pioglitazone alone.
Pregnancy and lactiation implications
Adverse events observed in animal studies. Severe hypoglycemia lasting 4-10 days noted in infants born to mothers taking a sulfonylurea at the time of delivery. Insulin is the drug of choice for control of diabetes mellitus during pregnancy. Not recommended during lactation.
Unlabeled use
Contraindications
Hypersensitivity to glimepiride, any component of the formulation, or sulfonamides. Diabetic ketoacidosis (with or without coma).
Warnings and precautions
Possible severe hypoglycemia (increased risk with strenuous exercise, alcohol ingestion, insufficient caloric intake, or use in combination with other oral antidiabetic agents (e.g. rosiglitazone, metformin)). Possible loss of glycemic control during periods of stress (e.g. fever, trauma, infection, surgery). Efficacy of therapy may decrease over time (secondary failure). Addition of metformin or insulin to therapy may be required. Possible increased risk of adverse effects in the elderly (due to age-related decreases in renal function). Oral hypoglycemic drugs may be associated with increased cardiovascular mortality as compared to treatment with diet alone or diet plus insulin. Hypoglycemia. Sulfonamide allergy (use in sulfonamide allergy contraindicated). Caution if patient is exposed to stress (fever, trauma, infection, surgery).
