Chemistry

Griseofulvin: C~17~H~17~ClO~6~. Mw: 352.77. (1) Spiro[benzofuran-2(3H),1′-[2]cyclohexene]-3,4′-dione, 7-chloro-2′,4,6-trimethoxy-6′-methyl-, (1’S-trans)-; (2) 7-Chloro-2′,4,6-trimethoxy-6’β-methylspiro[benzofuran-2(3H),1′-[2]cyclohexene]-3,4′-dione. CAS-126-07-8.

Pharmacologic Category

Miscellaneous Antifungals. (ATC-Code: D01AA08; D01BA01).

Mechanism of action

Antifungal antibiotic produced by Penicillium. Inhibits fungal cell mitosis at metaphase. Binds to human keratin making it resistant to fungal invasion.

Therapeutic use

Treatment of dermatophytoses of skin, scalp, and nails, including Tinea barbae (ringworm of bearded areas of face and neck), Tinea capitis (scalp ringworm), Tinea corporis (ringworm of the body), Tinea cruris (jock itch; groin ringworm), Tinea pedis (athlete’s foot, foot ringworm), and Tinea unguium (onychomycosis; nail ringworm) caused by Trichophyton, Microsporum, or Epidermophyton floccosum.

Pregnancy and lactiation implications

Animal studies have shown decreased spermatogenesis, as well as embryotoxic and teratogenic effects with griseofulvin. There are no adequate, well-controlled studies in pregnant women. Use during pregnancy is contraindicated.

Unlabeled use

Contraindications

Hypersensitivity to griseofulvin or any component of the formulation. Severe liver disease. Porphyria (interferes with porphyrin metabolism). Pregnancy.

Warnings and precautions

Hypersensitivity cross-reaction between penicillins and griseofulvin possible. May cause fetal harm when administered to pregnant women. Hypersensitivity reactions (e.g. rash, urticaria, erythema multiforme-like reactions, angioneurotic edema) reported. Photosensitivity reactions reported. Lupus erythematosus may be aggravated if photosensitivity reaction occurs. Should not be used for treatment of pityriasis (tinea) versicolor, cutaneous Candida infections, or systemic fungal infections (including blastomycosis, candidiasis, chromoblastomycosis, coccidioidomycosis, cryptococcosis, histoplasmosis, or sporotrichosis). Use should also be avoided in treatment of bacterial infections, including actinomycosis or nocardiosis. Contraindicated in hepatocellular failure.
