Chemistry

Permethrin: C~21~H~20~Cl~2~O~3~. Mw: 391.29. (1) Cyclopropanecarboxylic acid, 3-(2,2-dichloroethenyl)-2,2-dimethyl-, (3-phenoxyphenyl)methyl ester; (2) m-Phenoxybenzyl (±)-3-(2,2-dichlorovinyl)-2,2-dimethylcyclopropanecarboxylate; (3)(±)-3-Phenoxybenzyl 3-(2,2-dichlorovinyl)-2,2-dimethylcyclopropanecarboxylate. CAS-52645-53-1 (1987).

Pharmacologic Category

Skin and Mucous Membrane Agents; Scabicides and Pediculicides. Topical Antiparasitic Agent. Scabicidal Agent. (ATC-Code: P03AC04).

Mechanism of action

Inhibits sodium ion influx through nerve cell membrane channels in parasites resulting in delayed repolarization and thus paralysis and death of pest.

Therapeutic use

Single-application treatment of infestation with Pediculus humanus capitis (head louse) and its nits or Sarcoptes scabiei (scabies). Prophylactic use during epidemics of lice.

Pregnancy and lactiation implications

Use during pregnancy only if clearly needed. Unknown whether distributed into milk (use caution).

Unlabeled use

Contraindications

Hypersensitivity to pyrethyroid, pyrethrin, chrysanthemums, or any component of the formulation. Lotion contraindicated for use in infants <2 months of age.

Warnings and precautions

Treatment may temporarily exacerbate skin irritation (itching, redness, and swelling). For external use only. May cause breathing difficulty or asthmatic episode in susceptible individuals. 5% cream contains formaldehyde 0.1% as preservative, which may cause contact dermatitis.
