Chemistry

Ranitidine Hydrochloride: C~13~H~22~N~4~O~3~S HCl. Mw: 350.86. 1,1-Ethenediamine, N-[2-[[[5-[(dimethylamino)methyl]-2-furanyl]methyl]thio]ethyl]-N’-methyl-2-nitro-, monohydrochloride. CAS-66357-59-3.

Pharmacologic Category

Other Antihistamines. Antiulcer Agents and Acid Suppressants; Histamine H~2~-Antagonists. (ATC-Code: A02BA02).

Mechanism of action

Competitive inhibition of histamine at H~2~-receptors of gastric parietal cells, which inhibits gastric acid secretion, gastric volume, and hydrogen ion concentration.

Therapeutic use

Short-term and maintenance therapy of duodenal ulcer, gastric ulcer, gastroesophageal reflux, active benign ulcer, erosive esophagitis, and pathological hypersecretory conditions. As part of a multidrug regimen for H. pylori eradication.

Pregnancy and lactiation implications

Use with caution during pregnancy. Enters breast milk (use with caution in nursing women).

Unlabeled use

Recurrent postoperative ulcer, upper GI bleeding, prevention of acid-aspiration pneumonitis during surgery, and prevention of stress-induced ulcers.

Contraindications

Hypersensitivity to ranitidine or any component of the formulation.

Warnings and precautions

Long-term therapy may be associated with vitamin B~12~ deficiency. Reversible confusional states (rare), linked to use. Increased age (>50 years) and renal or hepatic impairment thought to be associated. Relief of symptoms does not preclude presence of gastric malignancy. Use with caution in hepatic impairment. Avoid use in history of acute porphyria (may precipitate attacks). Use with caution in renal impairment.
