Pharmacologic Category

Skin and Mucous Membrane Agents; Antibacterials. Pleuromutilin Antibiotic. Topical Antibiotic. (ATC-Code: D06AX13).

Mechanism of action

Primarily bacteriostatic. Inhibits normal bacterial protein biosynthesis by binding at unique site (protein L3) on ribosomal 50 S subunit. Prevents formation of active 50 S ribosomal subunits by inhibiting peptidyl transfer and blocking p-site interactions at this site.

Therapeutic use

Treatment of impetigo caused by susceptible strains of S. pyogenes or methicillin-susceptible S. Aureus.

Pregnancy and lactiation implications

Teratogenic effects not observed in animal studies. Excretion in breast milk unknown (use with caution).

Unlabeled use

Contraindications

Hypersensitivity to retapamulin or any component of the formulation.

Warnings and precautions

Skin irritation might occur. Prolonged use may result in fungal or bacterial superinfection, including C. difficile-associated diarrhea and pseudomembranous colitis. For external use only; not for intranasal, intravaginal, ophthalmic, oral, or mucosal application. For treatment of impetigo covering up to 100 cm^2^ total area in adults, or 2% of total body surface area in children.
