Spectinomycin Hydrochloride: C~14~H~24~N~2~O~7~ 2HCl 5H~2~O. Mw: 495.35. (1) 4H-Pyrano[2,3-b][1,4]benzodioxin-4-one, decahydro-4a,7,9-trihydroxy-2-methyl-6,8-bis(methylamino)-, dihydrochloride, pentahydrate; (2) Decahydro-4a,7,9-trihydroxy-2-methyl-6,8-bis(methylamino)-4H-pyrano[2,3-b][1,4]benzodioxin-4-one dihydrochloride pentahydrate. CAS-22189-32-8; CAS-21736-83-4 (anhydrous); CAS-1695-77-8 (spectinomycin)(1962).

Pharmacologic Category

Miscellaneous Antibacterials; Aminocyclitols. (ATC-Code: J01XX04).

Mechanism of action

Inhibits protein synthesis in susceptible bacteria by binding to 30S ribosomal subunits. Usually bacteriostatic in action. In vitro spectrum of activity includes Neisseria gonorrhoeae and a wide variety of Gram-positive and Gram-negative bacteria. Inactive against Treponema pallidum or Chlamydia trachomatis. Resistance to spectinomycin induced in vitro and spectinomycin-resistant strains of N. gonorrhoeae reported. Usually active against strains of N. gonorrhoeae resistant to fluoroquinolones.

Therapeutic use

Gonorrhea and associated infections.

Pregnancy and lactiation implications

Can be used for treatment of gonorrhea in pregnant women who are hypersensitive to cephalosporins. Unknown whether distributed into milk (use with caution).

Contraindications

Hypersensitivity to spectinomycin.

Warnings and precautions

Anaphylaxis or anaphylactoid reactions reported. Use with caution in history of allergies. Spectinomycin-resistant strains of N. gonorrhoeae reported.
