Chemistry

Terbinafine: C~21~H~25~N. Mw: 291.43. (1) 1-Naphthalenemethanamine, N-(6,6-dimethyl-2-hepten-4-ynyl)-N-methyl-, (E)-; (2)(E)-N-(6,6-Dimethyl-2-hepten-4-ynyl)-N-methyl-1-naphthalenemethylamine. CAS-91161-71-6; CAS-78628-80-5 (hydrochloride)(1987).

Pharmacologic Category

Antifungals; Allylamines.

Mechanism of action

Synthetic allylamine derivative which inhibits squalene epoxidase, a key enzyme in sterol biosynthesis in fungi. This results in deficiency in ergosterol within fungal cell wall and in fungal cell death.

Therapeutic use

Active against most strains of Trichophyton mentagrophytes, Trichophyton rubrum. May be effective for infections of Microsporum gypseum and M. nanum, Trichophyton verrucosum, Epidermophyton floccosum, Candida albicans, and Scopulariopsis brevicaulis. Onychomycosis of toenail or fingernail due to susceptible dermatophytes. Treatment of tinea capitis. Antifungal for treatment of tinea pedis (athlete’s foot), tinea cruris (jock itch), and tinea corporis (ringworm). Tinea versicolor.

Pregnancy and lactiation implications

Avoid use in pregnancy. Enters breast milk, not recommended in lactation.

Contraindications

History of or allergic reaction to oral terbinafine.

Warnings and precautions

Use caution in patients sensitive to allylamine antifungals (e.g. naftifine, butenafine); cross sensitivity to terbinafine may exist. Although rare, Stevens-Johnson syndrome and toxic epidermal necrolysis reported with oral use. Although rare, pancytopenia and neutropenia reported with oral use. Rare cases of hepatic failure (including fatal cases) reported following oral treatment. Not recommended for use in active or chronic liver disease. Although rare, changes in ocular lens and retina reported with oral use. Use of oral therapy not recommended in hepatic cirrhosis (clearance reduced by approximately 50%). Precipitation or exacerbation of cutaneous or systemic lupus erythematosus observed with oral therapy. Use of oral therapy not recommended in renal dysfunction (CrCl ≤50 mL/minute).
