Chemistry

Thiabendazole: C~10~H~7~N~3~S. Mw: 201.25. (1) 1H-Benzimidazole, 2-(4-thiazolyl)-; (2) 2-(4-Thiazolyl)benzimidazole. CAS-148-79-8 (1962).

Pharmacologic Category

Antihelmintics. (ATC-Code: D01AC06; P02CA02).

Mechanism of action

Inhibits helminth-specific mitochondrial fumarate reductase.

Therapeutic use

Treatment of strongyloidiasis, cutaneous larva migrans, visceral larva migrans, dracunculiasis, trichinosis, and mixed helminthic infections.

Pregnancy and lactiation implications

Cleft palate and skeletal defects observed in some animal studies. There are no adequate, well-controlled studies in pregnant women. Excretion in breast milk unknown (not recommended in nursing women).

Unlabeled use

Cutaneous larva migrans (topical application).

Contraindications

Hypersensitivity to thiabendazole or any component of the formulation. Not for use as prophylactic treatment of enterobiasis (pinworm) infestation.

Warnings and precautions

May cause CNS depression. Erythema multiforme and Steven’s-Johnson syndrome reported (including fatalities). Ophthalmic changes may occur and persist >1 year. Use with caution in anemia. Not suitable treatment for mixed infections with Ascaris. Use with caution in dehydration, in hepatic impairment (jaundice, cholestasis, parenchymal liver damage occurred), in malnutrition, and in renal impairment. Safety and efficacy limited in children <14 kg (30 lb).
